purchase from Southeastern Medical Supply, Inc.
We are NOT Medicare Providers & will not accept or process any Medicare
Claims.
Simply complete these documents and submit them to our Insurance
department and we will take the next step. Please note
that these documents are in an Adobe .pdf format and you will
need Adobe reader to view and/or print them.
